Stabilization Case Manager - Community Based Shelter Program

FamilyAid Boston is seeking to hire a Stabilization Case Manager to work in our Community Based Shelter Program.

Summary of the Position:

The Stabilization Case Manager will provide a minimum of monthly outreach to families that exit from shelter to a range of destinations from permanent, long-term subsidies, to short-term and shallow subsidies, to market rate units. The Case Manager works with each family on his or her caseload for 12 months to provide support in this transition. The objective is to build on the skills, strategies and goals developed in shelter, and to provide the individualized supports and interventions needed to assist each family in maintaining safe and affordable housing and increasing family stability. 

Duties & Responsibilities: 

  • Assess short and long-term stabilization and housing needs and, together with family, develop a plan to address those needs. Review and update the plan at regular intervals.
  • Collaborate with shelter staff, employment and housing staff to understand family’s experience in shelter, their strengths, and potential barriers to stability.
  • Help family establish residential stability and strengthen connections to the neighborhood, schools, medical providers, employment services, and other resources as needed.  
  • Assist family with all aspects of managing tenancy, including communication and collaboration with landlords.  Provide advocacy, consumer education, and crisis management when necessary to prevent housing disruption.
  • Work with families around budgeting and financial management and referrals to community programs as appropriate.
  • Provide advocacy for basic needs and other mainstream benefits including food, medical care, education, etc.
  • Routine home visits to support and assist the family.
  • Accompany family to housing interviews and appointments, and support the family in meeting requirements associated with housing subsidy.
  • Monitor client progress and complete regular documentation and reporting.
  • Participate in all staff, team and clinical meetings.
  • Participate in supervision and professional development.
  • Assume other responsibilities as requested by supervisor. 
  • Administrative responsibilities as required by program needs.


  • Bachelor’s degree and experience in social services preferred.
  • Valid MA Driver’s License.
  • Ability to work with families/individuals from diverse social, cultural and ethnic backgrounds in a culturally sensitive, team-oriented, and strength-based approach.
  • Demonstrated ability to work in an office, field-based, and community-based settings with a diverse, homeless population.  
  • Bilingual Spanish strongly preferred

Work Schedule:

Full-time, 40 hours. Ability to work some evenings as needed to accommodate client schedules.
